Hey all! Erin here from Printable Crush with another FREE Printable! How is it already time to do back to school preparation? I am getting ready in the one way I know how…by creating printables! I wanted to do something a little different with this Back to School Questionnaire and make it a coloring page, too. It will keep the kids busy for a little while longer, plus it’s one more way to make a questionnaire unique to each child.
My kids are still in elementary school, so they’ll get a kick out of these. I have a 3rd grader who just got her ears pierced, so she’s very grown up right now. She kind of thrives on filling out quizzes about herself. My ALMOST 1st grader (I’m in denial that he’s not in Kindergarten anymore) is really getting the hang of reading and writing, so I think filling out this Back to School Questionnaire will be great practice before he gets back in school. The transition from Kindergarten to 1st grade is a little rough, right?

I think kids are going to love coloring this in and filling it out, and you’ll love reading it later! Such a fun keepsake!
If you’d like, you can also attach a picture on the back. I’m one of those moms that takes a First Day of School picture every year. I even created some Hand Lettered Chalkboard Signs because it’s one thing that’s really important to me to document those milestones. I love looking back each year and seeing how much they’ve grown (although that’s a little bittersweet…because they just grow way too fast)!
